Assisted Living by Sholt offers a fun family environment, private rooms, medical transportation and community outings, and Internet access and training are provided.

Services

We are licensed at the Directed Care Services level and may provide:

• Activity Programs that ensure the resident is afforded daily rehabilitative, social and recreational activities customized to the individual needs.
• Administration of resident's personal funds account not to exceed $500, upon written authorization by a resident or representative.
• Food Services Program and menus are developed in accordance with The Food Guide Pyramid, USDA Center for Nutrition Policy and Promotion, Home and Garden Bulletin Number 252, Revised October 1996, incorporated by reference and on file with the Department and the Office of the Secretary of State and approved by a Registered Dietitian. Therapeutic and other specialized diets are provided when physician ordered.
• Housekeeping services
• Laundry Service is provided or access to washer and dryer depending on resident's Service Plan.
• Safety Features include smoke detectors that are hard wired, a property alarm system that is monitored by a third party 24/7, and, most importantly, a 24-hour responsive staff.
• Transportation - non-wheelchair medical transportation is coordinated for medical appointments and provided for individual outings.

The goal of these services is to promote a resident's independence, dignity, choice, self-determination, and the resident's highest physical, cognitive, and functional capability.
